
Your pet's health history is important for us to know. Please come to your visit prepared with answers to these questions that help us assess your pet's health.
Has your pet's appetite increased or decreased since the last visit?
Has your pet's thirst or urination increased since the last visit? If so, please bring a urine sample with you to your visit.
Has your pet had difficulty urinating or defecating? If so, please bring a urine and stool sample to your visit.
Has your pet had any urinary or fecal accidents in the house recently? If so, please bring a urine and stool sample to your visit.
Has your pet had problems with vomiting?
Has your pet had problems with diarrhea? If so, please bring a stool sample to your visit.
Has your pet been coughing?
Has your pet been sneezing? If so has there been nasal discharge and what is the color?
Have you noticed any lumps or bumps on your pet? If so, jot down the location and/or mark the area with a permanent marker so it is easy for your veterinarian to find.
Has your pet been itchy or is there a rash?
Has your pet been limping or having difficulty walking?
Has your pet been less active than usual?
Has your pet been exhibiting any troublesome behaviors?
What diet is your pet eating? How much food is being fed?
